Job Description
Responsibilities
Evaluate patients with speech, language, cognitive, and swallowing disorders.
Develop and implement individualized plans of care using evidence-based practices.
Monitor patient progress and adjust treatment plans as appropriate.
Evaluate treatment effectiveness and coordinate referrals when necessary.
Educate and counsel patients, families, caregivers, and interdisciplinary team members.
Communicate patient progress, barriers, and treatment plans with the healthcare team.
Complete discharge summaries and maintain accurate clinical documentation.
Document all services in the electronic medical record according to regulatory and company requirements.
Accurately record treatment minutes.
Participate in interdisciplinary meetings, care conferences, utilization reviews, and required training.
Obtain necessary equipment and therapy supplies as needed.
Supervise Clinical Fellowship (CFY) candidates and student Speech-Language Pathologists when applicable.
Adhere to ASHA standards, state practice acts, ethical guidelines, and professional regulations.
Minimum Requirements
Minimum of 1 year of Speech-Language Pathologist experience (Required)
Long-Term Care (LTC) or Rehabilitation experience (Required)
Active Oklahoma Speech-Language Pathologist License (Required)
Certificate of Clinical Competence (CCC-SLP) (Required)
New graduates are welcome to apply if they have completed a Level II clinical experience (observation-only experience does not qualify).
License Requirement
Active Oklahoma SLP License
