• Valid Hawaii RN license
• Proven clinical nursing experience, preferably in home health or community-based settings
• Strong patient advocacy and case management skills
• Excellent communication and interpersonal skills
• Ability to coordinate multidisciplinary care and navigate complex cases
• Knowledge of regulatory and compliance standards in home health care
• Critical thinking and problem-solving abilities
• Ability to work independently and manage time effectively

Nice to Have Skills:

Preferred Education and Experience:

• Bachelor of Science in Nursing (BSN) preferred
• Minimum of 2 years of recent home health or outpatient nursing experience

Other Requirements:

• Valid Hawaii driver’s license and reliable transportation
• Willingness to travel within the service area as needed
• Current BLS certification; additional certifications in case management or palliative care are advantageous
• Ability to adapt to a flexible schedule to accommodate patient needs
