Pro Rehab, an established Rehab company with over 35 years of successful business is hiring a Therapist (PT, OT, SLP) to join the team at Maple Lawn Senior Care, in Fulda, MN! Therapist Responsibilities:

Assess & Evaluate Patients: Conduct comprehensive evaluations to identify physical, cognitive, communication, and/or swallowing needs based on discipline-specific scope of practice.
Develop & Implement Treatment Plans: Create and carry out individualized treatment plans and rehabilitation programs to address functional deficits and promote independence.
Provide Therapeutic Interventions: Deliver skilled, hands-on therapy and therapeutic activities, including exercises, manual techniques, adaptive strategies, assistive device training, and communication or swallowing interventions as appropriate.
Educate & Support Patients, Families, & Staff: Provide education such as but not limited to pain management, fall prevention, home safety, coping strategies, communication techniques, and strategies to improve daily functioning and quality of life.
Monitor Progress & Document Care: Track patient outcomes, document evaluations and progress, and modify treatment plans based on patient response and clinical judgment.
Collaborate with Interdisciplinary Teams: Work closely with physicians, nurses, therapists, educators, and other healthcare professionals to ensure coordinated, comprehensive patient care.

Physical Therapist Qualifications:

Must be a graduate from an Accredited Therapy Program and have a Bachelor's Degree
Must possess current licensure for MN, ND or IA
Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care.
Must be capable of performing the essential functions of the job, with or without reasonable accommodations.
Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.
