• Developing and implementing individualized treatment plans in collaboration with physicians, patients, and families.
• Conducting comprehensive patient assessments and continuously evaluating treatment effectiveness.
• Educating patients and families about treatment goals, techniques, and discharge instructions.
• Documenting all treatments, evaluations, and patient interactions in accordance with departmental protocols.
• Providing discharge planning and referrals to community resources to support ongoing recovery.
• Acting as a resource for staff and physicians regarding rehabilitation services and equipment.
• Upholding ethical standards, maintaining patient confidentiality, and demonstrating professionalism at all times.
• Engaging in ongoing professional development, mandatory education, and team meetings.
• Ensuring adherence to safety, infection prevention, and hospital policies.

Required skills include:

• Effective communication and interpersonal skills.
• Customer service orientation.
• Ability to work collaboratively within a team.
• Basic computer proficiency.
• Current licensure as a Physical Therapist and BLS certification.

Preferred qualifications:

• Previous experience in a hospital or outpatient rehabilitation setting.
• Familiarity with electronic medical records systems.
• Additional certifications or specialized training in orthopedics, neurology, or other relevant areas.

Educational and experience prerequisites:

• Graduate from an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate).
• Valid licensure as a Physical Therapist.
• BLS certification upon hire.

Additional requirements:

• Ability to work day shifts (Monday through Friday) with two weekend days per month.
• Hourly pay rate of $46.90.
• Willingness to adhere to all Wellstar health system policies and standards.
• Must possess all required licenses and certifications upon employment.
