Therapy/Rehabilitation/PT Home Health:Duration:13 Weeks Start:03/02/2026 Shift: Day 5x8-Hour (08:00 - 17:00)Description: Physical Therapist in Home Health provides personalized rehabilitation care to patients recovering from injury, surgery, or managing chronic conditions in their homes. They assess mobility, strength, and functional abilities, then develop and implement individualized treatment plans. PTs educate patients and caregivers on exercises, safety, and pain management techniques. They monitor progress, adjust therapies as needed, and collaborate with physicians and other healthcare providers to optimize recovery and independence.
