Critical Access Hospital outpatient/inpatient, home visits with swing bed program in a rural community
The PT will evaluate, plan, and implement individualized treatment programs to help patients improve mobility, reduce pain, and regain independence. This role works collaboratively with providers and care teams across acute, outpatient, and/or long-term care settings.
