MUST HAVE IDAHO LICENSE AT TIME OF SUBMISSION Hospital: West Valley Medical Center Location: Caldwell, Idaho Address: 1717 Arlington Ave. Caldwell, ID 83605 Unit Name: Behavior Health Unit Building: Main Hospital Cost Center: 76114 General Information Unit Specifics: Number of Beds : 22 Patient Ratios : RN 8:1, SW 10:1 Unit staffing description- (unit secretaries and tech availability) : two RNs, two techs for 12 or more patients, two SW scheduled five days a week, two days will be only one SW Type of equipment : locked unit Documentation system: Meditech Scheduling requirements: SW scheduled 5 days a week, 8-hours a day Floating policy: BHU staff does not float. Patient Experience requirements: Treatment team, PSA, PHQ9, BHA, two groups daily Hospital Specifics- Certifications: RN, BHT, LCSW, LMSW Hiring profile Requested Skill Set: Must have: Must have an Idaho license whether RN or SW Advanced skills needed: None Procedures performed on unit: No procedures held on unit. May take patients off unit for ECT. Certifications Required: Would prefer an LCSW, but will utilize an LMSW for our needed MHP position. Must be licensed within the state of Idaho. RNs must be licensed. Dress Code: (Required uniform) SW business casual RN Navy blue tops and bottoms or Galaxy blue tops and bottoms or black bottoms Unit Guidelines/Policies: Please attach unit specific policies and guidelines. Leapfrog A standing several years FRIST award recipient in our volunteer services Any additional information that would entice a worker to select your unit for their assignment: Consider items such as new equipment or technology, tenure and experience of staff, schedule options, etc. We have an amazing team to work with. Our psych providers are great. They are present on the unit and are easily accessible as needed. #TB_HC
