Responsibilities
Provide high-quality nursing care to patients in various local settings.
Participate in ongoing professional development and training programs.
Achieve performance targets and contribute to team success.
Collaborate with healthcare teams to develop patient care plans.
Educate patients and families on health management and wellness.
Monitor and document patient progress and treatment outcomes.
Utilize pipelines to assess and enhance patient care strategies.
Maintain accurate and up-to-date patient records and documentation.
Adhere to all safety and regulatory guidelines in patient care.
Provide feedback to improve nursing practices and patient outcomes.
Engage in community outreach to promote health awareness.
Experience/Qualifications
Active RN license in the state of practice.
Commitment to ongoing professional development and training.
Proven ability to meet performance targets and contribute to team success.
Experience in providing nursing care in community or local settings.
Ability to collaborate effectively with multidisciplinary healthcare teams.
Strong communication skills for patient and family education.
Familiarity with safety and regulatory guidelines in nursing.
Capability to monitor and document patient progress effectively.
Proficiency in using healthcare documentation systems.
Experience in community outreach and health promotion activities.
