Responsibilities
Assist licensed nurses with daily resident care and safety tasks.
Monitor residents’ vitals and report changes to supervising Nurse.
Engage in continuous learning to progress toward advanced nursing roles.
Uphold quality and compliance with health regulations and facility policies.
Maintain accuracy in scheduling and leveraging care pipelines for tasks.
Document care activities promptly in resident records and logs.
Communicate effectively with residents, families, and the care team.
Assist with activities of daily living (ADLs) as directed by nurses.
Maintain a clean and safe work environment following infection control standards.
Support discharge planning and transition processes for residents.
Participate in team meetings to share progress and identify improvement opportunities.
Assist with transportation and mobility support as required.
Experience/Qualifications
Certified Nursing Assistant (CNA) license or equivalent required.
Assist with ADLs and daily resident care under supervision of licensed nurses.
Monitor vitals and report changes promptly to supervising nurse.
Accurate documentation of care activities in resident records and logs.
Adhere to infection control standards and maintain a clean, safe environment.
Excellent communication with residents, families, and teammates.
Willingness to learn and progress toward advanced nursing roles.
Support discharge planning and transition processes for residents.
Strong teamwork with care team; participate in regular meetings.
Ability to manage transport and mobility support as needed.
