Responsibilities

• Assist licensed nurses with daily resident care and safety tasks.

• Monitor residents’ vitals and report changes to supervising Nurse.

• Engage in continuous learning to progress toward advanced nursing roles.

• Uphold quality and compliance with health regulations and facility policies.

• Maintain accuracy in scheduling and leveraging care pipelines for tasks.

• Document care activities promptly in resident records and logs.

• Communicate effectively with residents, families, and the care team.

• Assist with activities of daily living (ADLs) as directed by nurses.

• Maintain a clean and safe work environment following infection control standards.

• Support discharge planning and transition processes for residents.

• Participate in team meetings to share progress and identify improvement opportunities.

• Assist with transportation and mobility support as required.

Experience/Qualifications

• Certified Nursing Assistant (CNA) license or equivalent required.

• Assist with ADLs and daily resident care under supervision of licensed nurses.

• Monitor vitals and report changes promptly to supervising nurse.

• Accurate documentation of care activities in resident records and logs.

• Adhere to infection control standards and maintain a clean, safe environment.

• Excellent communication with residents, families, and teammates.

• Willingness to learn and progress toward advanced nursing roles.

• Support discharge planning and transition processes for residents.

• Strong teamwork with care team; participate in regular meetings.

• Ability to manage transport and mobility support as needed.
