Local Travel Physical Therapist (PT) - South of San Jose, CAContract Length: 13 WeeksWeekly Compensation: Up to $3,160 (Based on Experience) Connected Health Care is seeking an experienced Physical Therapist (PT) for a local travel assignment in San Jose, California. This is a great opportunity to work with a reputable healthcare facility while staying close to home. Whether you're a seasoned traveler or new to contract work, we provide unmatched support every step of the way.Job Description:
- Evaluate, plan, and implement individualized treatment plans for patients*
- Use evidence-based practices to restore function, relieve pain, and improve mobility*
- Educate patients and families on home exercise programs and prevention strategies*
- Collaborate with physicians, nursing staff, and other therapists to ensure quality care*
- Document patient progress and update treatment plans accordingly*
- Maintain compliance with all local, state, and federal regulations
- Degree in Physical Therapy from an accredited program Required
- Current California PT License Required
- BLS Certification Required
- Minimum of 1 year of clinical experience preferred
