Note: All RTO must be approved prior to contract approval. Every weekend commitment. Must have own transport.

Responsibilities

Deliver direct nursing care to residents following individualised care plans and facility policies.
Administer medications and treatments safely within LPN scope of practice.
Monitor, document, and promptly report changes in resident condition to the supervising RN and provider.
Collaborate with CNAs and the interdisciplinary team to support resident goals and care plan compliance.

Requirements

Active, unencumbered LPN licence in the applicable state.
Minimum 1 year SNF, LTC, or post-acute experience preferred.
Current BLS/CPR certification.
Reliable, detail-oriented, and committed to compassionate resident care.
