JOB RESPONSIBILITIES- Provide direct patient care under the supervision of registered nursing staff
- Administer medications and treatments within LPN scope of practice
- Monitor patient health status and report changes to clinical team
- Assist with activities of daily living (ADLs) including hygiene, mobility, and comfort care
- Perform basic clinical procedures such as vital signs monitoring and wound care
- Document patient care activities accurately and timely
- Support care planning and implementation of nursing interventions
- Ensure patient safety and adherence to infection control protocols
- Collaborate with interdisciplinary healthcare team members
- Active Licensed Practical Nurse (LPN) license required
- Graduate of an accredited practical nursing program
- Ability to work assigned 12.5-hour shifts
- Strong clinical assessment and communication skills
- Ability to perform physical duties including standing, lifting, and repositioning patients
- Compliance with facility onboarding and credentialing requirements
- Valid state or federal identification required
- COVID-19 vaccination or approved exemption required
- Prior experience in long-term care or skilled nursing preferred
