JOB OVERVIEW
We are seeking a skilled and dependable Local Licensed Practical Nurse (LPN) for a 15-week contract assignment in Concord, New Hampshire. The LPN will provide high-quality nursing care to residents in a skilled nursing and rehabilitation setting while ensuring patient safety, comfort, and continuity of care during overnight shifts.
This role involves administering medications and treatments, monitoring patient conditions, documenting clinical information, and collaborating with the healthcare team to support positive patient outcomes.
KEY RESPONSIBILITIES
• Provide direct nursing care under the supervision of an RN• Administer medications and prescribed treatments accurately• Monitor and document patient vital signs and clinical status• Assist with wound care and infection control procedures• Respond to resident care needs during overnight shifts• Report changes in patient condition to supervising staff• Maintain accurate and timely clinical documentation• Collaborate with interdisciplinary healthcare teams• Follow facility policies, procedures, and safety standards
REQUIREMENTS
• Active Licensed Practical Nurse (LPN) license (New Hampshire or compact eligible)• Minimum 1 year of LPN experience preferred in skilled nursing settings• Ability to work overnight 12-hour shifts• Strong clinical judgment and communication skills• Ability to work in a fast-paced healthcare environment• Negative COVID-19 test result required prior to assignment start or proof of full vaccination• Commitment to patient-centered and safe care delivery
