The LPN will support residents with clinical needs while ensuring safety, comfort, and quality of care.
This role involves working as part of a multidisciplinary team to deliver consistent care across assigned shifts in a structured healthcare environment. Shift Details Shift Time: 18:00 06:30 Number of Shifts: 3 shifts per week Key Responsibilities Provide direct nursing care under RN supervision Monitor and document patient conditions Administer medications and treatments as prescribed Assist with activities of daily living Report changes in patient status promptly Maintain accurate clinical documentation Follow infection control and safety protocols Collaborate with healthcare team members Requirements Active Licensed Practical Nurse (LPN) license Experience in long-term care or rehabilitation preferred
