OverviewLicensed Practical Nurse (LPN) – Long Term Care (Post-Acute Skilled Nursing Rehab) in Middlebury, Vermont, United States. This is a travel, temporary assignment starting September 24, 2026 for 5 weeks. Weekly pay ranges from $1,405 to $1,418 per week, with 0.0 guaranteed hours. Shift: 12-hour day shifts, 3 per week. This assignment is in a long-term care / nursing home setting focused on post-acute care.Key Highlights- Travel assignment: 5 weeks- 12-hour day shifts, 3x per week- Post-acute/skilled nursing rehab unit- Vermont LPN license required at time of submission- EPIC EMR experience within the past 2 years required- Minimum 1 year of clinical experience; prior travel experience required- Dementia-capable care certification preferred; IV certification preferred- Unit may require on-site collaboration with LNA/CNA/PCT and unit clerkResponsibilities- Provide direct patient care to residents on the post-acute care unit under RN/charge nurse supervision- Administer prescribed medications and treatments per physician orders; monitor and document patient responses- Perform or assist with phlebotomy, EKGs, and transports as permitted; manage IV pumps with appropriate certification- Assist residents with activities of daily living and participate in restorative/rehabilitative activities- Monitor for changes in condition, implement fall prevention and infection control measures, and report to the care team- Maintain accurate documentation in the EPIC EMR and communicate changes to the care teamRequirements- Active Vermont LPN license at time of submission- BLS certification required- EPIC EMR experience within the past 2 years- Minimum 1 year of clinical experience; prior travel experience required- Dementia-capable care certification preferred- IV certification preferred- Ability to work in a post-acute skilled nursing rehab setting; willingness to work 12-hour day shifts- Comprehensive credentialing and background checks required (nationwide and county searches, FACIS3, VCIC) within specified timelines- Reliable transportation required; public transit may not be accessible- Must be able to work all holidays as assignedBenefits and Support- Assignment-specific credentialing and onboarding support (background checks, FACIS3, VCIC)- Bi-weekly orientation sessions on Mondays during the assignment- Dedicated recruiter and travel-support resources from Healthforce Healthcare LLC- Onsite collaboration with the care team and access to the patient care workflow (EPIC)- Clear path to start within unit protocols and post-acute care guidelinesFind more opportunitiesIf you’d like to view other openings available, please apply at https://healthforce.applytojob.com/applyPAY DISCLAIMERPlease note: Pay rates and guaranteed hours are estimates and may vary.
