OverviewLicensed Practical Nurse (LPN) – Skilled Nursing – Wray, Colorado, United States. Travel assignment begins 12/07/2026 for weeks with 40 guaranteed hours per week. Evening shift, 15:00–23:00, five days per week in a skilled nursing facility.Key Highlights- Weekly pay: $1,445–$1,493 per week- Guaranteed hours: 40 per week- Contract duration: weeks (travel assignment)- Shift: Evenings (15:00–23:00)- Start date: 12/07/2026- Setting: Skilled Nursing FacilityResponsibilities- Provide direct patient nursing care to residents under supervision of a registered nurse- Administer prescribed medications and document administration per policy and procedure- Monitor vital signs, assess patient condition changes, and escalate as needed- Perform routine nursing procedures, wound care, ADLs assistance, and infection control- Maintain accurate health records and contribute to care planning and interdisciplinary communication- Promote patient safety, assist with transfers and mobility, and uphold a clean, organized care environmentRequirements- Active LPN license (state-specific licensure as required)- Current CPR/BLS certification for healthcare providers- Ability to work the stated evenings shift (15:00–23:00)- Experience in a skilled nursing or long-term care setting preferred- Strong communication and teamwork skills- Willingness to travel for a travel assignmentBenefits and Support- 24/7 recruiter and travel support- Dedicated assignment support throughout the engagement- Credentialing and onboarding guidance- Access to nationwide travel opportunities (where available)Please note: Pay rates and guaranteed hours are estimates and may vary.
