OverviewPhysical Therapist – Skilled Nursing Facility travel assignment in Carmichael, California. This opportunity starts on 11/30/2026 for a 13-week contract with guaranteed hours. Weekly pay ranges from $2,587 to $2,593 per week, with a 35-hour minimum per week. Shift is 5x8 days, with hours from 08:00 to 16:30. The role is in a skilled nursing facility setting.Key Highlights- Weekly pay: $2,587–$2,593 per week- Guaranteed hours: 35 per week- Contract duration: 13 weeks- Shift: 5x8 days, 08:00–16:30- Start date: 11/30/2026- Work setting: Skilled Nursing FacilityResponsibilities- Perform comprehensive PT evaluations upon admission and during regular reassessments to establish baseline function, identify goals, and guide the plan of care for residents with diverse geriatric conditions.- Develop and implement individualized, evidence-based treatment plans targeting mobility, strength, balance, gait quality, activities of daily living independence, and fall risk reduction; adjust as progress occurs.- Provide hands-on therapy sessions including therapeutic exercises, gait training, neuromuscular re-education, manual techniques, equipment use, and appropriate modalities per plan of care.- Monitor clinical outcomes and progress; document with timely, accurate records; communicate progress to the interdisciplinary team, families, and physicians; participate in care conferences.- Educate residents and family caregivers on home exercise programs, safety strategies, fall prevention, energy conservation, and equipment use to support continuity of care after SNF discharge.- Ensure compliance with facility policies, state and federal regulations, documentation standards, infection control, and patient confidentiality.Requirements- Availability to start 11/30/2026 and commit to a 13-week travel assignment; flexibility to adapt to assignment needs.- Ability to work the 08:00–16:30 shift on a 5x8 schedule and provide 35 hours per week; reliable attendance and timeliness.- Experience delivering physical therapy services to geriatric or skilled nursing facility populations; familiarity with SNF care processes and goals.- Strong documentation skills, proficiency with electronic medical records, and effective communication within an interdisciplinary team.- Willingness to comply with facility policies, regulatory requirements, and assignment-specific guidelines; ability to adapt to a SNF setting.Benefits and Support- Dedicated recruiter and travel support throughout the assignment- 24/7 support for clinical questions, credentialing, and scheduling needs- Onboarding and credentialing assistance to facilitate a swift start- Extension opportunities based on performance, patient needs, and facility demandFind more opportunitiesIf you would like to explore other openings available at Healthforce Healthcare LLC, please visit https://healthforce.applytojob.com/applyPAY DISCLAIMER RULE:Please note: Pay rates and guaranteed hours are estimates and may vary.
