Job Description

OverviewPhysical Therapist – Skilled Nursing Facility travel assignment in Albuquerque, NM, US. Start date 07/27/2026 for 13 weeks. Weekly pay range $2,264–$2,387 per week with guaranteed hours of 36. Shift is 08:00–16:30, five days per week. This is a travel - temporary assignment through Healthforce Healthcare LLC.Key Highlights- Weekly pay: $2,264–$2,387 per week- Guaranteed hours: 36 per week- Contract duration: 13 weeks- Shift: 08:00–16:30, five days per week- Start date: 07/27/2026- Setting: Skilled nursing facility travel assignmentResponsibilities- Conduct initial PT evaluations and develop individualized plans of care for adults in a skilled nursing setting.- Provide therapy interventions including therapeutic exercise, gait and balance training, ROM and strength work, and functional mobility tasks.- Monitor patient progress, document outcomes, and adjust the plan of care as needed in accordance with policy.- Collaborate with interdisciplinary team members (nursing, OT, SLP, physicians) to optimize patient outcomes and discharge planning.- Ensure patient and staff safety during treatment sessions and maintain infection control standards.- Educate patients and families on home exercise programs and strategies to promote independence.Requirements- Doctor of Physical Therapy (DPT) degree or equivalent- Active Physical Therapist license in the United States with eligibility to practice in New Mexico- Current CPR/BLS certification- Preferred: 1+ year of PT experience in geriatric or skilled nursing rehabilitation- Ability to work in a skilled nursing facility environment and participate in a 13-week travel assignment- Agencies must have a NM caregivers account to engage with the assignmentBenefits and Support- 36 guaranteed hours per week throughout the assignment- 13-week contract with structured schedule- Travel coordination and dedicated recruiter support from Healthforce Healthcare LLC- Credentialing and onboarding assistance to support a smooth start- On-call and on-site support during the assignment- NM caregiver account requirement noted for agency engagementFind more opportunitiesIf you’re exploring other openings from Healthforce Healthcare LLC, apply at https://healthforce.applytojob.com/apply to view additional travel PT roles and locations.PAY DISCLAIMER RULE:Please note: Pay rates and guaranteed hours are estimates and may vary.
