Job Title: Physical Therapist- Home Health/Hospice

Location: Grants Pass, OR

Contract: 3 months contract with contract extensions

Shift: Mon-Fri 8:00am- 16:30pm

Hours Per week: 40 hours per week

Description:Coverage Areas: Josephine County On Call/Weekend Requirement: n/aWill Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes What are the top responsibilities for this position? (Please be as detailed as possible): Physical Therapy Visits- SOC, PT eval, Recert, ROC, supervisory visits Must have qualifications/experience: Doctorate Physical TherapyWhat skills/attributes are nice to have, and will set a candidate apart? Home Health experience, HomeCare Homebase EMR experienceDress code/scrub color: business casual or any color of scrubs
