Overview
Job Description
- Patient Evaluation & Care Planning
- Evaluate patient functional status, physical therapy needs, and all body systems required for CMS documentation
- Collaborate with physicians to develop individualized plans of care
- Complete all patient evaluations and establish care plans within state-specific guidelines
- Report evaluation outcomes, goals, and frequency of care within 24 hours
- Treatment & Clinical Care
- Provide therapy services in accordance with physician orders and care plans
- Treat patients to relieve pain, restore function, and maintain optimal mobility using evidence-based techniques
- Monitor patient progress and adjust treatments as needed
- Observe and report changes in patient condition to physicians or nurse supervisors
- Patient & Caregiver Education
- Educate patients, families, and caregivers on therapy plans, goals, and functional limitations
- Provide guidance on exercises, mobility techniques, and overall health management
- Documentation & Compliance
- Complete accurate and timely OASIS and visit documentation per agency standards
- Maintain compliance with CMS requirements, policies, and clinical protocols
- Prepare progress notes for each patient visit and submit within required timelines
- Provide discharge summaries or status reports at least every 60 days
- Collaboration & Leadership
- Collaborate with interdisciplinary care teams to ensure coordinated patient care
- Participate in staff meetings and case conferences
- Supervise Physical Therapist Assistants (PTAs) and home health aides as applicable
- Provide in-service education and mentorship as requested
- Operational & Field Responsibilities
- Travel to patient homes across the assigned coverage area and nearby regions as needed
- Maintain schedule flexibility to meet patient care needs, including potential weekend rotations/holiday coverage
- Adhere to all safety, infection control, and therapy practice standards
- Active and current Physical Therapist (PT) license
- Home Health experience required
- OASIS documentation experience required
- Current CPR certification
- Strong clinical, documentation, and communication skills
