• Patient Care & Intervention: Implement therapeutic interventions, including therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities according to the established Plan of Care.
• Assessment & Monitoring: Observe, assess, and document patient responses to treatments, progress toward goals, and functional outcomes.
• Documentation: Accurately document all visits in the Electronic Medical Record (EMR) system (HCHB) in a timely manner, using measurable, objective, and individualized clinical content that complies with Medicare and agency standards.
• Care Coordination: Promptly communicate patient status changes, safety concerns, lack of progress, or needs for Plan of Care updates to the supervising Physical Therapist.
• Interdisciplinary Collaboration: Collaborate with RNs, OTs, STs, and case management teams to ensure coordinated, high-quality patient care.
• Safety & Compliance: Follow infection control, safety, and universal precautions during all home visits; maintain licensure and regulatory compliance according to Florida practice acts and agency policies.
• Education: Educate patients and caregivers on home exercise programs, fall prevention, assistive device usage, and functional safety in the home environment.
• Efficiency: Manage assigned visits efficiently to support productivity expectations and geographic coverage needs.

• Education: Graduate of an accredited Physical Therapist Assistant (PTA) program.
• Licensure: Current, active Physical Therapist Assistant license in the State of Florida.
• Certification: CPR Certification (BLS – Healthcare Provider).
• Experience: Minimum of 1 year of PTA experience.
• Technical Skills: Proficiency and experience with the Homecare Homebase (HCHB) EMR system.
