Position Summary
The Physical Therapy Assistant (PTA) provides high-quality, patient-centered physical therapy services in a home health setting under the direction of a licensed Physical Therapist. This role supports patients in achieving optimal functional independence while adhering to established care plans and organizational standards.
Key Responsibilities
- Deliver physical therapy treatments as outlined in the Plan of Care developed by the supervising Physical Therapist
- Assist patients in improving mobility, strength, and functional independence in their home environment
- Accurately document patient visits, progress, and outcomes using Homecare Homebase (HCHB) system
- Communicate regularly with the Physical Therapist and interdisciplinary team regarding patient status and progress
- Educate patients and caregivers on therapeutic exercises, safety practices, and use of assistive devices
- Maintain compliance with all regulatory, clinical, and organizational policies
- Travel between patient homes within assigned coverage area
Required Qualifications
- Associate degree from an accredited Physical Therapy Assistant program
- Active Physical Therapy Assistant license (state-specific requirement)
- Home Health experience required
- Experience using Homecare Homebase (HCHB) strongly preferred
- Current Basic Life Support (BLS) certification
- Valid driver’s license, reliable transportation, and auto insurance
Preferred Qualifications (Nice-to-Have)
- Strong clinical assessment and patient care skills in a home health environment
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Experience working with diverse patient populations
- Familiarity with home health regulations and compliance standards
Work Environment & Additional Details
- Travel-based role covering multiple locations within assigned territory
- Mileage reimbursement provided between patient visits
- No on-call or weekend requirements
