Shift Schedule: Mon–Fri, 8:00 AM – 4:30 PM
Coverage Areas: N West Arkansas. Counties: Sebastian, Logan, Crawford, Franklin, Scott, Yell
Mileage reimbursed between visits, from first patient to last patient: Yes
- SOC visits, Recert, Evaluations, ROC’s, and subsequent skilled visits
- Perform initial, recertification, and discharge assessments in patients’ homes
- Conduct comprehensive OASIS assessments accurately and on time.
- Monitor patient responses and adjust interventions as clinically indicated.
- Perform medication reconciliation at each visit.
- Educate patients and caregivers on medication purpose, dosing, side effects, and compliance.
- Communicate changes in patient condition and obtain updated physician orders.
