Pride Health is hiring a Physical Therapist to support our client's patients in Mountlake Terrace, WA, with coverage in King County and Snohomish County (Seattle to Mill Creek). On Call/ weekend Requirement: NoMileage: Reimbursement provided (from first patient to last patient)Job Responsibilities:
- Perform comprehensive OASIS assessments using HCHB (Homecare Homebase), including Start of Care (SOC), recertification, 30-day reassessments, and discharge evaluations in accordance with physician orders and agency protocols.
- Conduct skilled physical therapy evaluations assessing strength, mobility, balance, pain, gait, and functional limitations to develop individualized Plans of Care (POC).
- Provide skilled physical therapy interventions during routine and follow-up (revisit) appointments to maximize patient independence and functional outcomes.
- Perform re-evaluations to assess patient progress, modify treatment plans, and update functional goals as clinically appropriate.
- Supervise and collaborate with Physical Therapist Assistants (PTAs), ensuring delegated treatments are completed appropriately and in compliance with regulatory standards.
- Educate patients and caregivers on home exercise programs, fall prevention, safety techniques, mobility training, and disease management strategies.
- Coordinate care with physicians, interdisciplinary team members, caregivers, and support staff to ensure continuity of care and optimal patient outcomes.
- Complete timely, accurate, and compliant clinical documentation within HCHB in accordance with Medicare, OASIS, and organizational standards.
- Conduct discharge visits including final assessments, patient/caregiver education, and transition planning to support safe continuation of care.
Qualifications:
- Graduate of an accredited Physical Therapy program (master’s or higher).
- Active, unrestricted Physical Therapist (PT) license.
- Minimum 1 year of clinical experience (home health experience/ HCHB required).
- Valid driver’s license and reliable transportation.
