OverviewRegistered Nurse – Case Management in a travel assignment based in Cape Coral, FL. Start date 12/07/2026 for 13 weeks. Weekly pay range $1,653–$1,689 with guaranteed 36 hours. Shift: 12-hour evenings. Work in a hospital setting with potential to float to sister campuses as needed. The role centers on acute care case management, coordinating patient needs, discharge planning, and collaboration with the multidisciplinary care team to support safe transitions and appropriate resource utilization.Key Highlights- Weekly pay: $1,653–$1,689 per week- Guaranteed hours: 36 per week- Contract duration: 13 weeks- Shift: 12-hour evening (12x3)- Start date: 12/07/2026- Setting: hospital-based travel assignment with potential to float to sister campuses as neededResponsibilities- Conduct comprehensive case management for acute care patients, including initial assessment, risk stratification, and development of individualized care plans.- Coordinate discharge planning with patients, families, social work, rehab services, and post-acute providers to ensure safe and timely transitions.- Collaborate with physicians, nurses, and care teams to optimize treatment plans, utilization, and resource allocation.- Monitor patient progress, identify barriers to care, and advocate for appropriate services and supports to meet clinical and social needs.- Document plans, actions, and outcomes accurately in the electronic health record; ensure compliance with regulatory and facility-specific policies.- Participate in interdepartmental rounds and care conferences; be prepared to float to sister campuses as needed to support staffing and patient care demands.Requirements- Active registered nurse license; Nursys must be on profile.- BLS for healthcare providers required.- 2+ years of acute care case management experience preferred.- Degree accreditation must be on profile or candidate must have 5 years of case management experience.- Ability to work in a hospital setting and willingness to float to sister campuses as needed.Benefits and Support- Guaranteed hours- 13-week assignment- Floating to sister campuses as needed- Travel assignment in a hospital settingFind more opportunitiesIf you’d like to see other openings from Healthforce Healthcare LLC, visit https://healthforce.applytojob.com/apply to view additional travel opportunities and apply.Please note: Pay rates and guaranteed hours are estimates and may vary.
