OverviewRegistered Nurse – Case Management, travel assignment in a hospital setting located in Fort Myers, FL, United States. Start date: 11/30/2026 for a 13-week assignment. Weekly pay: $2,183–$2,294 per week. Guaranteed hours: 40 per week. Shift: Day shift, 8:00 a.m. to 4:40 p.m., Monday through Friday. This is a travel temporary assignment through Healthforce Healthcare LLC.Key Highlights- Weekly pay: $2,183–$2,294 per week- Guaranteed hours: 40 hours per week- Contract duration: 13 weeks- Shift: Day shift, 8:00 a.m.–4:40 p.m., Monday–Friday- Start date: 11/30/2026- Work setting: Hospital-based case management role- Floating required: travelers may float to other units or sister campuses- Submission requirements: all RTO requests and demographic information must be included at submittalResponsibilities- Manage complex case loads for inpatient and high-acuity patients, coordinating care across disciplines- Develop, implement, and re-evaluate comprehensive care plans to optimize outcomes and LOS- Collaborate with physicians, social workers, and other care teams to coordinate discharge planning and post-acute needs- Assess patient needs, monitor utilization, and ensure appropriate resource use while complying with regulatory standards- Document care coordination activities thoroughly in the medical record and communicate updates to the care team- Provide patient and family education on discharge instructions, medications, and follow-up plansRequirements- Active RN license in the state of Florida; Nursys verification must be on profile- BLS certification required- Minimum 2 years of Case Management experience in a hospital/acute care setting- Degree accreditation or 5 years of related experience required- Ability to float to other units or sister campuses as needed- Radius rule: within 50 miles of perm address- Willingness to participate in a travel assignment with on-site collaboration and rapid credentialingBenefits and Support- Dedicated support throughout the assignment- 24/7 recruiter and travel support- Nationwide assignments- Fast credentialingFind more opportunitiesTo explore additional openings from Healthforce Healthcare LLC, please visit https://healthforce.applytojob.com/applyPAY DISCLAIMER RULE:Please note: Pay rates and guaranteed hours are estimates and may vary.
