Job Description
OverviewRegistered Nurse – Case Management in Glendale, California. Travel - temporary assignment in a hospital setting. Start date: 10/12/2026 for an expected 13-week contract. Weekly pay ranges from $2,766 to $2,900 per week. Shift: Days, 8 hours per day (40 hours per week).Key Highlights- Weekly pay: $2,766–$2,900 per week- Contract Weeks: 13- Shift: Days, 8 hours per day (40 hours per week)- Assignment type: Travel - temporary- Start date: 10/12/2026- Setting: Hospital-based care coordinationResponsibilities- Provide excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program.- Act as a liaison between patient, family, nurse, physicians, multidisciplinary team and patient’s healthcare benefactor to optimize outcomes.- Serve as a consultant to the healthcare team on specific patient items.- Coordinate a continuum of care for a defined population from pre-admission through post-discharge.- Ensure patients and families proceed efficiently through the course of hospitalization.Requirements- Active registered nurse license in California (CA)- Current Basic Life Support (BLS) certification- Prior case management or care coordination experience preferred- Ability to work in a hospital setting on a travel assignment- Excellent communication and collaboration skills with multidisciplinary teams- Strong clinical assessment and care planning abilitiesBenefits and Support- Dedicated recruiter and travel support throughout the assignment- Assistance with onboarding and clinical orientation- Travel coordination and potential housing guidance- Opportunities for assignment extension or additional openingsFind more opportunitiesInterested in other roles from Healthforce Healthcare LLC?
