Job Description

OverviewRegistered Nurse – Case Manager, Everett, WA. Start date 05/26/2026 for an 8-week travel assignment in an acute care setting. Weekly pay ranges from $2,090 to $2,176, with guaranteed hours of 72 per two weeks (approximately 36 hours weekly). Shift: 4x10-hour day shifts. This role focuses on discharge planning and care coordination within a hospital setting and involves working with an interdisciplinary team to support transitions to the next level of care.Key Highlights- Weekly pay: $2,090–$2,176 per week- Guaranteed hours: 72 hours per 2 weeks- Contract length: 8 weeks- Shift: 4x10 days- Start date: 05/26/2026- Setting: acute care hospital; case management / discharge planning- Onboarding: orientation (30 hours) plus pre-start modules (approximately 16 hours)- Travel assignment with pre-approved time off: 1 request allowed- Floating: may float across hospital areas to support patient flow- Holiday coverage: must commit to 3 of the 5 major holidaysResponsibilities- Manage discharge planning and care coordination for diverse patient populations across the hospital system.- Collaborate with the interdisciplinary team, including Social Services, to arrange appropriate post-acute care and care transitions.- Maintain accountability for a caseload of approximately 25–30 patients, coordinating care plans and ensuring timely patient throughput.- Document care coordination activities in EPIC and participate in rounds and care conferences.- Identify barriers to discharge, coordinate with ancillary services, and educate patients and families about post-acute needs and resources.- Ensure compliance with hospital policies, patient safety standards, and regulatory requirements.Requirements- Licensure: RN license in Washington state; pending WA license accepted per job notes. The role may also consider MSW candidates with relevant case management experience.- Certifications: BLS required.- Experience: minimum 1 year of acute care experience with discharge planning / care coordination; diverse case management background preferred.- Education: RN degree; or MSW degree as applicable to the role.- Skills: strong care coordination, discharge planning, and documentation in EPIC; ability to manage a multi-unit workflow and float as needed.- Additional: ability to work in a high-volume hospital environment; willingness to work 4x10-hour shifts and to cover scheduled holidays; references (2) covering at least 1 year of experience within the last 3 years; readiness to disclose any background or Nursys findings at submittal.Benefits and Support- Orientation: 30 hours- Pre-start modules: non-billable, approximately 16 hours- Parking: facility-paid- Dedicated assignment support from Healthforce Healthcare LLC- Background check and reference process included- Interdisciplinary collaboration with Social Services for care transitions- Pre-approved time off: 1 requestFind more opportunitiesNot this exact opening?
