Job Description

OverviewSocial Worker - Case Management (MSW) needed for a travel assignment in Everett, Washington. Start date: 06/08/2026. Duration: 12 weeks. Weekly pay: $1,707–$1,809 per week. Guaranteed hours: 72 hours per 2-week pay period. Shift: 5x8 Days. Setting: acute care environment with adult and geriatric patients. This assignment requires a WA license in-hand and a strong focus on discharge planning and care coordination within an interdisciplinary team.Key Highlights- Weekly pay: $1,707–$1,809 per week- Guaranteed hours: 72 hours per 2-week pay period- Contract duration: 12 weeks- Shift: 5x8 Days- Start date: 06/08/2026- Orientation: 40 hoursResponsibilities- Provide Medical Social Work (MSW) services for adults and geriatrics, including psychosocial assessments and care planning- Lead discharge planning and care coordination to support safe transitions to next levels of care- Collaborate with the interdisciplinary team (PT, interpreter services, social services, pharmacy, transportation) to optimize patient outcomes- Manage a caseload typical of 18–24 patients, with weekend caseloads up to ~30, and document in the charting system- Utilize guardianship processes, advance directives, DME referrals, and connections to community resources- Support guardianship and high-risk interventions as part of comprehensive care management across Med-Surg, ED, and related areasRequirements- Active Washington clinical social work license in-hand (LICSW, LSWAIC, LASW, LSWAA)- BLS certification- Master’s degree in Social Work (MSW)- Minimum 1 year clinical experience with acute care/discharge planning and care coordination- Experience with guardianship, community resources, DME referrals, adult protective services, and needs assessment- Proficiency with EPIC or equivalent electronic charting; ability to work across a fast-paced environment and float as needed- Ability to work within an acute care setting with adult and geriatric populations- Reference requirements: two references covering at least one year within the last three years- Pre-start requirements: pre-start modules (approx. 16 hours) and completion reported in Workramp- Scheduling constraints: willing to work every other weekend; holiday coverage commitment to 3 of the listed holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day)- RTO planning: include all requested time off at submittal; standard client/offers window noted as 5 days- Compliance: background/Nursys verification may be required at submittalBenefits and Support- 40-hour orientation included- Pre-start modules (approx. 16 hours) required; completion tracked in Workramp- 24/7 recruiter and travel support- Dedicated support throughout the assignment- Housing assistance and nationwide assignment opportunities- Fast credentialing and extension opportunitiesFind more opportunitiesTo explore additional openings available at Healthforce Healthcare LLC, please apply through https://healthforce.applytojob.com/applyPlease note: Pay rates and guaranteed hours are estimates and may vary.
