OverviewPosition: Social Worker (Medical Social Worker) — Sitka, Alaska. Start date 10/05/2026 for a 13-week travel assignment. Weekly pay range: $2,456-$2,625 per week. Setting: hospital environment.Key Highlights- Weekly pay: $2,456-$2,625 per week- Guaranteed hours: 40 hours per week- Contract duration: 13 weeks- Shift: 5x8 days- Start date: 10/05/2026- Travel assignment in a hospital setting- All-inclusive rate; housing and flights not providedResponsibilities- Work with the inpatient multidisciplinary care team to assess biopsychosocial needs of patients admitted for medical care.- Lead discharge planning in collaboration with inpatient care coordinators; identify barriers to discharge and arrange community resources, DME needs, and transportation.- Complete swing-bed assessments at admission for subacute rehabilitation, ensuring patient/family understanding and participation requirements.- Facilitate or co-facilitate patient/family care conferences to review illness, plan of care, and navigation of the health care system.- Support patient and family decision-making, including discussion of advance directives, power of attorney, and POLST where appropriate.- Identify community resources to assist during hospitalization and recovery; assist with housing, financial, legal, and basic living needs; coordinate with agencies as needed.- Maintain accurate records, document in a timely manner, and collaborate with SEARHC providers to ensure continuity of care within scope of responsibilities.- Ensure confidentiality and culturally sensitive communication; participate in required trainings and multidisciplinary team meetings.Requirements- Education: BSW or MSW in social services required- Experience: Minimum 2 years of experience in a hospital setting in transitional care or equivalent social work capacity- Certifications/Licenses: BLS preferred; flu vaccine required- Credentials: Fingerprint-based background clearance required; prints must be completed within 1 week of offer- Additional: Must be at least 21 years old- Location restrictions: Travelers must reside at least 100 miles away from the assignment location- Skills: Strong communication, case management, documentation, and coordination with multi-disciplinary teams; ability to work independently in a fast-paced hospital environmentBenefits and Support- Weekly pay range: $2,456-$2,625 per week- Guaranteed hours: 40 hours per week- Contract duration: 13 weeks- Shift: 5x8 days- Travel assignment with no housing or flights provided (all-inclusive rate)- Onboarding and credentialing support through Healthforce Healthcare LLCFind more opportunitiesIf you’d like to explore other openings available through Healthforce Healthcare LLC, apply at https://healthforce.applytojob.com/applyPAY DISCLAIMER RULE:Please note: Pay rates and guaranteed hours are estimates and may vary.
