Job Description

Covenant Case Management Services is seeking a compassionate, dedicated, and structured individual or family to serve as an Alternative Family Living (AFL) Provider. This unique, full-time opportunity involves opening your heart and your private home to provide a safe, supportive, and nurturing residential environment for an individual with intellectual and developmental disabilities (I/DD) under the NC Innovations Waiver.

As an unlicensed AFL Host, you will not just be providing a service- you will be integrating this individual into your daily life, helping them build independence, and supporting them both at home and within the community.

Responsibilities:

  • Habilitation & Daily Living: Provide consistent, one-on-one support with daily living activities, including personal care, meal preparation, and medication administration.

  • Community Integration: Actively accompany and support the individual in community outings, social activities, and appointments to foster inclusion and independence.

  • Home Administration: Maintain a clean, safe, and positive home environment that meets health and safety standards.

  • Documentation & Compliance: Follow Covenant Case Management Services policies, including tracking goals and documenting daily logs as required by the Innovations Waiver.

Qualifications:

  • Home Requirement: Must own or rent a home with a dedicated, private bedroom for the individual that can pass standard environmental safety checks.

  • Education: High school diploma or equivalent.

  • Documentation: Valid North Carolina driver’s license, reliable transportation, and auto insurance.

  • Screening: Ability to pass a comprehensive criminal background check, healthcare registry check, and driving record check.

  • Experience: Experience working with individuals with I/DD is preferred, but we will provide comprehensive training for the right compassionate and patient provider.

Work Schedule:

  • Full-Time / Residential: This is a live-in, 24/7 residential contract. (AFL Provider model).
