Position Details

• Position: Travel Social Worker

• Location: Near Winner, SD

• Setting: Long-Term Care (LTC)

• Contract Length: 13 Weeks

• Schedule: Days | 5x8s

• Guaranteed Hours: 40 Hours per Week

• Pay: Up to $45/hour (based on experience)

• Start Date: ASAP

Job Responsibilities

• Complete psychosocial assessments for residents upon admission and as needed.

• Develop and implement individualized care plans addressing residents' emotional, social, and psychosocial needs.

• Provide counseling, emotional support, and crisis intervention for residents and their families.

• Coordinate discharge planning and facilitate transitions of care.

• Connect residents and families with community resources and support services.

• Advocate for resident rights and promote quality of life.

• Collaborate with physicians, nursing staff, therapy teams, and administration to ensure comprehensive resident care.

• Maintain accurate documentation in accordance with state and federal regulations.

• Assist with care conferences, family meetings, and regulatory compliance initiatives.

Education & Qualifications

• Bachelor's Degree in Social Work (BSW) required.

• Master's Degree in Social Work (MSW) preferred.

• Current South Dakota Social Work license (or eligibility to obtain prior to start) preferred, as required by state regulations.

• Previous experience in Long-Term Care, Skilled Nursing, Rehabilitation, or Geriatric Care preferred.

• Strong communication, organizational, documentation, and interpersonal skills.

• Ability to work independently while collaborating effectively with interdisciplinary teams.
