Upperline Health is the nation's largest provider dedicated to lower extremity, wound and vascular care. Founded in 2017 with the ambitious goal of changing specialty care, Upperline Health delivers a more efficient path for patients to receive consistent and effective treatment for chronic illnesses.

Triage is temporary.

Treatment is transformative.

With over 130 podiatric medical and surgical clinics in 7 states, Upperline Health providers coordinate patients' care among a team of specialists - podiatric physicians and surgeons, advanced practice providers, care navigators, pharmacists, dieticians, and social workers for integrated treatment that addresses patients' immediate and long-term health needs.

We put patients at the center of value-based care.

  • Order, perform, and interpret relevant labs, imaging, and diagnostic tests.
  • Educate patients and families on disease progression, medication adherence, technology use (e.g., CGMs, insulin pumps), and lifestyle modification.
  • Document visits and treatment plans thoroughly and accurately in our electronic medical record system (EMR).
  • Serve as a key medical leader in interdisciplinary case reviews and care planning.
  • Support continuous quality improvement, clinical protocol development, and practice growth.
  • Maintain all appropriate licensure and certifications, including CME and board requirements

  • Passion for serving high-risk, underserved, or chronically ill populations
  • Excellent communication and interpersonal skills to foster meaningful patient and team relationships
  • Familiarity with or willingness to learn value-based care models and interdisciplinary collaboration
  • Experience with CGMs, insulin pumps, and diabetes technologies is preferred but not required
