Position Summary:Heart To Serve Home Care is seeking high-caliber CNAs who demonstrate strong dementia care skills, professionalism, and emotional intelligence. Our caregivers provide relationship-centered, concierge-level care designed to enhance safety, independence, and quality of life for clients living with Alzheimer’s and memory-related conditions.
Primary Responsibilities: Provide personal care including bathing, grooming, dressing, toileting, and hygiene support Deliver safe ambulation and transfer assistance Implement structured dementia communication and redirection strategies Monitor behavioral, cognitive, and functional changes Support fall prevention and environmental safety Assist with meal preparation, hydration, and companionship Maintain accurate and professional documentation Follow individualized care plans and collaborate with clinical leadership
Required Qualifications: Active Indiana CNA license in good standing Demonstrated experience supporting clients with dementia or Alzheimer’s disease CPR certification and TB screening Strong communication and observation skills Professional, dependable, and compassionate approach to care Reliable transportation Ability to pass comprehensive background screening
