Your day as a CNA In-Home Care Coordinator/Scheduler

As a Full-Time CNA In-Home Care Coordinator/Scheduler at Senior Partners, you will play a vital role in assisting our Administrator with staffing and care coordination needs for our dedicated 24/7 service. This exciting position allows you to utilize your expertise to ensure that our clients receive top-notch care around the clock. You’ll be the backbone of our operations, orchestrating schedules and ensuring the right caregivers are in the right place at the right time. To thrive as a Full-Time CNA In-Home Care Coordinator/Scheduler at Senior Partners, you'll need a blend of vital skills and experience to ensure smooth operations for our caregiving staff and senior clients. A current CNA certification for Utah is essential, along with a high school diploma or equivalent. Your background in home health care scheduling will be invaluable—ideally, you’ve spent at least a year navigating care coordination software, such as WellSky Personal Care or a similar platform.

Excellent verbal and written communication skills are a must as you’ll be interacting with clients, caregivers, and team members daily. Flexibility is key; you'll be quick to respond to same-day scheduling changes and adept at juggling multiple priorities. Strong problem-solving abilities and a calm, empathetic demeanor will help you support both clients and caregivers effectively, ensuring everyone receives the quality care they deserve while remaining committed to our mission.

Knowledge and skills required for the position are:

  • As an In-home care Scheduler and CNA Care Coordinator you will play a critical role in coordinating visits for care partners and senior clients. Your attention to detail and communication skills will ensure our clients receive the care they need on time and in compliance with care plans and regulatory requirements.
  • Key Responsibilities:
  • Oversee all agency Scheduling tasks for caregiving staff in accordance with client needs and specific care plans
  • Communicate clearly with clients and staff regarding visit times and any changes
  • Maintain accurate visit logs and scheduling data in the EMR (e.g. WellSky Personal Care or equivalent)
  • Respond quickly to same-day scheduling changes visit cancellations or care giver availability issues
  • Working shifts that you are unable to cover
  • Work closely with care partners to provide support coaching an ongoing caregiver training
  • Oversee the hiring process for care partners and manage orientation/onboarding
  • Conduct on-site client/caregiver visits every six months and document findings
  • Facilitate client/care partner introductions with every new client/care partner
  • Qualifications:
  • Current CNA Certification for Utah
  • High School diploma or equivalent
  • Minimum 1 year of Scheduling in a home health or healthcare setting and proficiency in using care coordination software systems
  • Excellent verbal and written communication skills
  • Ability to work independently in a fast-paced environment and manage multiple priorities
  • Strong problem solving skills with a calm and empathetic communication style
  • Comfortable operating in a fast-paced high-demand environment while maintaining attention to detail
