Shift: Monday - Friday (8am-5pm) Weekends: Some weekend coverage is required

Call: Some call may be scheduled, but only on an emergency basis

Requirements:-Must have OASIS (Outcome and Assessment Information Set) Experience-Home Health and Hospice Experience required-Experience traveling from patient to patient for home visits-Must be able to manage and adjust to changes in schedule-Must have drivers license in good standing-BLS required-EMR system used is HealthWyse (Careficient) -Outpatient care-Facility does their own Background Check as well
