Legacy House of Southern Hills is seeking a compassionate, organized, and dedicated Wellness Nurse (LPN or RN) to join our exceptional senior living team. In this rewarding role, you will help support the health and wellness of our assisted living and memory care residents by managing chronic conditions, providing personalized care, and building strong relationships with residents and their families.

Key Responsibilities
As our Wellness Nurse, you will:
- Monitor and manage residents' chronic medical conditions
- Conduct health assessments and ongoing wellness evaluations
- Communicate regularly with residents, families, and healthcare providers
- Provide medication oversight and health education
- Support residents' overall physical and emotional well-being
- Maintain accurate documentation and regulatory compliance
- Promote a safe, compassionate, and resident-centered care environment
Qualifications
To succeed in this role, you should have:
- Active Nevada LPN or RN license
- Previous patient care or senior living experience preferred
- Strong communication and clinical assessment skills
- Excellent organizational abilities
- Compassionate, patient-focused mindset
- Ability to thrive in assisted living and memory care settings
- Schedule: Tuesday–Saturday | 9:00 AM – 5:00 PM
