Lund’s Regional Partnership Program (RPP) seeks a dedicated and driven Case Manager to support families, create meaningful change, and address substance use challenges in our communities. RPP Case Managers work out of local DCF Family Services offices to improve the well-being and permanency outcomes of children affected by parental substance misuse. This position is split between two DCF offices, working approximately 20 hours at the St. Johnsbury office and 20 hours at the Morrisville office. In close collaboration with DCF staff, RPP Case Managers play a vital role at the front end of child protection, screening for problematic substance use, linking parents to appropriate treatment services, and addressing barriers to successful engagement. By increasing access to and participation in treatment, RPP Case Managers provide support at the individual level and across the broader care system, ensuring families receive the help they need to thrive. This role conducts substance use screenings, coordinates referrals to assessment and treatment providers, and fosters treatment engagement by addressing transportation, childcare, or stigma barriers. Case managers provide consultation and training to DCF staff on substance use and treatment services while liaising between the treatment system and DCF Family Services.
