Qualifications Occupational Therapy License (Required) Responsibilities Provides Occupational therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to: Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment. Assesses for muscle strength, mobility, ROM—potential for rehab. Directing Occupational therapy treatment. Instructing patients and families/caregivers in the use and care of therapeutic appliances. Determining priority needs for Occupational therapy. Reporting to physician patient’s reaction to treatment or changes in condition. Initiates Occupational therapy program and instructs other personnel and/or family/caregiver members in certain phases of Occupational therapy with which they may work with a patient, as well as instructing them as to the goals of the Occupational therapy program for the patient by participating in case conferences. May train patient in the use of the prosthetic device. Identifies patient and family/caregiver needs for other home health services and refers as necessary. Prepares and submits clinical and progress summaries based on the attainment of goals. Participates in discharge planning for patients. Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy. Provides Occupational therapy consultation to home families/caregivers when indicated. Provides in-service education programs for nursing organization personnel as needed. Participates in the peer consultation process. Supervises Occupational therapy assistants according to organization policy and state regulations. Qualifications Possesses a degree from a program in Occupational therapy approved by an accredited organization. Licensed to practice as an Occupational therapist within the state. Two years of appropriate experience as an Occupational therapist.Community/home health experience. Demonstrates good verbal and written communication and organization skills. Occupational Therapist (OT)

Shifts Available: Full Time, Part Time & PRN

Service Areas: Northeast Houston, Lake Jackson, Conroe, Northwest Houston, Southwest Houston, Missouri City, Angleton

Responsibilities:

  • Facilitate rehabilitation of patients with emotional, physical, or mental disabilities.
  • Develop and implement occupational therapy care plans.
  • Coordinate care with home healthcare team.
  • Evaluate therapy needs and monitor and report patient progress and outcomes.
  • Educate patients and family members.
  • Prepare timely, complete, and accurate clinical notes.
  • Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications:

  • Occupational therapist in the state of Texas.
  • Home health experience highly preferred.
  • Knowledge of home health regulations and general healthcare compliance.
  • Able to handle physical workload.
  • Good driving record.
  • Experience with HCHB EMR (preferred but not required).
  • Effective communication and people skills.
