Will provide Physical Therapy services at Modesto PACE, and may provide support to our Modesto McHenry Specialty site as needed. Duties and Responsibilities Implement Physical Therapy program for PACE participants and day services recommended by the Physical Therapist. Access patient’s ability to perform therapeutic exercises and make recommendations and suggest modifications, based on patient abilities and patient population. Monitor and assist participant’s activity to ensure they are being completed correctly and offer encouragement. Provide therapies based on recommendations from Physical Therapist, specific to the individualized plan of care to decrease or eliminate disability, increase or maintain capability for independence, and develop function to a maximum level. Therapies to include, but not limited to: muscle, nerve joint and functional ability tests; treatment to relieve pain, develop or restore function; and assistance to achieve and maintain maximum performance using physical means such as exercise, heat, sound, water, light or electricity. Educate, guide, and assist patients and caregivers in the use of adaptive devices and therapeutic, creative and self-care activities for improving function. Evaluate and record the effectiveness of treatment, discuss findings with Physical Therapist and PACE team to assess recommendations to modify goals. Document all activities in patient's health record, ensuring appropriate signatures and treatments provided are present. Assists Physical Therapist to ensure required equipment and supplies for the physical therapy program are available, ordered if required, and follow-up as needed. Ensures of all regulations and standards are current and up to date, to make certain compliance with orders or directives issued by constituted government/regulatory agencies. Provide trainings to educate PACE staff and respective caregivers of therapeutic programs the PACE participant is participating in. Maintain and follow established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards. Maintain equipment and work area in a safe and clean condition. Attend meetings and participate on committees to share findings related to Physical Therapy when needed. Perform other related duties as assigned or requested. Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties. Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse. Other projects and duties as assigned. Physical Demands Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate. Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff. Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents. Must have high manual dexterity. Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling. Work Environment Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment. Subject to unpleasant odors. The noise level in the work environment is usually quiet to moderate. Education/Experience Requirements Minimum Qualifications Valid California Driver’s License, acceptable driving record and vehicle insurance. Knowledge of principles and methods for treatment and rehabilitation of disabilities and functional impairments. Patience and strong interpersonal skills. Excellent oral, written, and computer skills. Bilingual (English/Spanish) preferred. Ability to establish and maintain therapeutic relationships and to participate in a multi-cultural, multidisciplinary team. Education/Experience Must have one of the following qualifications: Associates Degree in Physical Therapy; Physical Therapy Degree earned outside of the US which has been evaluated to the US Equivalent to a degree in physical therapy. Active and unencumbered Physical Therapist Assistant licensed issued by the State of California Physical Therapy Board. Current BLS CPR Card certified by the American Heart Association. Minimum one (1) year experience providing personal care to a frail, disabled or geriatric population.

Job Description

The Physical Therapy Assistant (PTA) supports the mission, vision, and values of Golden Valley Health Centers Central Valley PACE participants by supporting a physical therapy program under the direction of a physical therapist for PACE patients, helping each achieve independence and safe living abilities.

Will provide Physical Therapy services at Modesto PACE, and may provide support to our Modesto McHenry Specialty site as needed.

Schedule is Monday – Friday, 8:00am – 5:00pm

Compensation:

$45.00 -$52.00 an hour

Duties and Responsibilities

  • Implement Physical Therapy program for PACE participants and day services recommended by the Physical Therapist.
  • Access patient’s ability to perform therapeutic exercises and make recommendations and suggest modifications, based on patient abilities and patient population.
  • Monitor and assist participant’s activity to ensure they are being completed correctly and offer encouragement.
  • Provide therapies based on recommendations from Physical Therapist, specific to the individualized plan of care to decrease or eliminate disability, increase or maintain capability for independence, and develop function to a maximum level.
  • Therapies to include, but not limited to: muscle, nerve joint and functional ability tests; treatment to relieve pain, develop or restore function; and assistance to achieve and maintain maximum performance using physical means such as exercise, heat, sound, water, light or electricity.
  • Educate, guide, and assist patients and caregivers in the use of adaptive devices and therapeutic, creative and self-care activities for improving function.
  • Evaluate and record the effectiveness of treatment, discuss findings with Physical Therapist and PACE team to assess recommendations to modify goals.
  • Document all activities in patient's health record, ensuring appropriate signatures and treatments provided are present.
  • Assists Physical Therapist to ensure required equipment and supplies for the physical therapy program are available, ordered if required, and follow-up as needed.
  • Ensures of all regulations and standards are current and up to date, to make certain compliance with orders or directives issued by constituted government/regulatory agencies.
  • Provide trainings to educate PACE staff and respective caregivers of therapeutic programs the PACE participant is participating in.
  • Maintain and follow established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
  • Maintain equipment and work area in a safe and clean condition.
  • Attend meetings and participate on committees to share findings related to Physical Therapy when needed.
  • Perform other related duties as assigned or requested.
  • Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
  • Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
  • Other projects and duties as assigned.

Physical Demands

  • Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate. 
  • Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
  • Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents. Must have high manual dexterity.
  • Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.

Work Environment

  • Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
  • Subject to unpleasant odors. The noise level in the work environment is usually quiet to moderate.

Education/Experience Requirements

Minimum Qualifications

  • Valid California Driver’s License, acceptable driving record and vehicle insurance.
  • Knowledge of principles and methods for treatment and rehabilitation of disabilities and functional impairments.
  • Patience and strong interpersonal skills.
  • Excellent oral, written, and computer skills.
  • Bilingual (English/Spanish) preferred.
  • Ability to establish and maintain therapeutic relationships and to participate in a multi-cultural, multidisciplinary team.

Education/Experience

  • Must have one of the following qualifications:
    • Associates Degree in Physical Therapy; 
    • Physical Therapy Degree earned outside of the US which has been evaluated to the US Equivalent to a degree in physical therapy.
  • Active and unencumbered Physical Therapist Assistant licensed issued by the State of California Physical Therapy Board.
  • Current BLS CPR Card certified by the American Heart Association.
  • Minimum one (1) year experience providing personal care to a frail, disabled or geriatric population.
