Job Description

Qualifications:

  • Master’s or Doctoral degree in Physical Therapy required
  • Minimum of two years’ experience working with and supporting people with intellectual disabilities
  • Current Physical Therapy license/certification for New York operation
  • Excellent verbal and written communication skills
  • Working knowledge of OPWDD regulations
  • Valid driver’s license required

Responsibilities:

  • Performs initial and ongoing physical therapy evaluations according to state standards as needed
  • Must become a direct Physical Therapist through EPIC Long Island’s Article 16 Clinic to provide direct therapy in the homes
  • Develops Plan of Treatment for each person based on initial and ongoing evaluations (i.e. Ambulation Guidelines, Fall Prevention Guidelines, Bathing Guidelines, Transfer Guidelines, Exercise Guidelines, etc.), including short and long term goals.
  • Recommends adaptive equipment (as needed) and ensures they remain in good/safe condition.
  • Implements Plan of Treatment on a consistent schedule and adjusts this schedule and plan according to the person’s progress.
  • Ongoing monitoring and documentation of person’s progress each time they are seen at the residence.
  • Trains management teams and staff in providing the appropriate care for the person supported. 
  • Collaborates and follows through on physical therapy recommendations from committees (i.e. HRC, IRC).
  • Ensures regulatory compliance with implementation of physical therapy services.
