Incorporates discharge planning into the treatment process in a timely manner. Follows departmental policies for documentation of medical records, billing, and continuing education. Requirements Graduate of an accredited physical therapy assistant program with either an Associate’s degree in science or applied science that is approved by American Physical Therapy Association, Inc. Current certification or license as required by state law. Possesses and maintains current CPR certification. Ability to be able to travel to different patients' homes. Discretion in dealing with sensitive and confidential information. Prior experience working in a home health setting is preferred, but not required.
