The Physical Therapist Assistant works under the supervision of a qualified Physical Therapist in accordance with the Home Health Plan of Care. The Physical Therapist Assistant works as part of the interdisciplinary home care team including the RN, PT, OT, COTA, SLP, MSW and Home Health Aides. PRIMARY RESPONSIBILITIES Provide physical therapy services to patients according to a home health plan of care and under the direction and supervision of a qualified Physical Therapist. This may include, but is not limited to: Performing physical therapy procedures in the patient’s home. Administering non-complex therapeutic exercises, therapeutic massage, traction, heat, light, cold, water and electrical modalities to patients with relatively stable conditions. Instructs, motivates, and assists patients in learning and improving functional activities. Teaches basic principles of body mechanics to patients and families. Instructs patients and family members on the proper use of prosthetic devices and in the use and care of therapeutic appliances. Observes patient’s progress and response to treatment and reports findings to the Physical Therapist. Reports changes in the patient’s condition to the supervising Physical Therapist. Modifies the Home Health Plan of Treatment, as authorized by the physician, after subsequent authorization, instruction and supervision by the Physical Therapist. Participates in educating the patient/family and in inservice programs. Participate in Home Health patient case conferences. Participate in Helping Hands Home Care Service’s Quality Assessment and Performance Improvement (QAPI) Program as requested. Confer with the Physical Therapist, attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care. Communicate the plan of care to other members of the home care team and patient/family and/or caregivers. Participate in meetings and other related activities as needed and directed. Prepare and submit clinical and progress notes in a timely manner and in accordance with Helping Hands Home Care Service policies, state and federal guidelines and regulations. Instruct patient and/or family/caregiver and other team members in the patient’s treatment regime as indicated. Meet or exceed delivery of Helping Hands Home Care Service service standards in a consistent fashion. Adhere to Helping Hands Home Care Service’s Code of Conduct. Comply with local, state and federal laws, Medicare Home Health regulations and established company policies and procedures. As a Physical Therapist, you’ll contribute to our success in the following ways:

  • Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care
  • Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.
  • Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
  • Conducts therapy practice within defined standards of care.
  • Directs the assistance of support staff to achieve patients’ functional goals.
  • Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.
  • Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community
  • Physically demanding, high stress environment
  • Full range of body motion including handling, lifting and transfer of patients 
  • Potentially work irregular hours including call hours if applicable

What is Required?

  • Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
  • Current, unrestricted PT license valid for the state of work
  • Minimum one (1) year of experience in a healthcare facility
  • Able and willing to travel within branch/office coverage area.
  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
  • Must be able and willing to travel 50%
  • Able to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

You will report to the Branch Director, Clinical Manager, Clinical Supervisor or designee.
