Job Description: Summary: Provides direct physical therapy, under supervision of the physical therapist, to patients in their residence. Implements plan of care. Adheres to the Code of Conduct and upholds the philosophy, vision, and core values of the organization and ensures the Christian Mission of EverTrue is realized so that “Older Adults Live Life to the Fullest”. Essential Duties and Responsibilities: 1.Assists Physical Therapist in developing and implementing individual treatment goals which reflect patient needs. 2.Assists Physical Therapist in developing an apt treatment plan on each patient. Reviews and consults with Physical Therapist frequently for needed changes. 3.Following review with Physical Therapist, communicated with other member of medical care team and personnel of other departments in providing for patient’s total needs. This includes recommending other ancillary service referrals. 4.Implements treatment plan as outlined by Physical Therapist and reports progress in medical record on each visit. 5.Provides support and education for patient and family members regarding patient’s PT needs. Notify, as appropriate, physician, home care nurse, PHYSICAL THERAPIST, promptly on any changes in patient’s physical or emotional condition, or any significant occurrences. 6.Attends and participates in case conferences and agency in-service programs. 7.Provides input in establishing department polices. 8.Promote awareness and utilization of home care services to other professionals, physicians, and the community. 9.Other duties as assigned. Summary:

Provides direct physical therapy, under supervision of the physical therapist, to patients in their residence.  Implements plan of care.  Adheres to the Code of Conduct and upholds the philosophy, vision, and core values of the organization and ensures the Christian Mission of EverTrue is realized so that “Older Adults Live Life to the Fullest”.

Essential Duties and Responsibilities:

1.Assists Physical Therapist in developing and implementing individual treatment goals which reflect patient needs.

2.Assists Physical Therapist in developing an apt treatment plan on each patient.  Reviews and consults with Physical Therapist frequently for needed changes.

3.Following review with Physical Therapist, communicated with other member of medical care team and personnel of other departments in providing for patient’s total needs.  This includes recommending other ancillary service referrals.

4.Implements treatment plan as outlined by Physical Therapist and reports progress in medical record on each visit.

5.Provides support and education for patient and family members regarding patient’s PT needs. Notify, as appropriate, physician, home care nurse, PHYSICAL THERAPIST, promptly on any changes in patient’s physical or emotional condition, or any significant occurrences.

6.Attends and participates in case conferences and agency in-service programs.

7.Provides input in establishing department polices.

8.Promote awareness and utilization of home care services to other professionals, physicians, and the community.

9.Other duties as assigned.

Qualifications, Knowledge, Skills and Abilities:
