Coordinate with others to assure transition assistance to individuals residing in a nursing facility, hospital, regional treatment center or licensed supervised living facility who request or are referred for assistance. Manage any appeals to level of care and eligibility determinations related to assessments you’ve completed. Participate in the county’s multidisciplinary long-term care consultation/certified assessor team. Authorize and enter PCA services and rates in the Medicaid Management Information System (MMIS).   • Assesses any relationships of the patient's medical and nursing needs in the home setting, financial resources, and available community resources.
