Shift: Days / 8:30a-5p, 8x5, 40hrs per week, M-F

Length: 13 weeks

Requirements:

• Active DC SW license
• At least 1 year of hospital case management (discharge planning) experience
• BLS
• LICSW
• Knowledge of Cerner

• Must live 50 miles away
