We are hiring a Social Services Professional to help support clients' well-being and independence by providing social work consultation; coordinating client care, as well as develops and facilitates therapeutic group activities.

Our Social Services Professional:

  • Provides information to referral sources and potential clients by receiving intake inquiries and giving tours.

  • Identifies clients' service needs by gathering and completing intake information and working with the client, caregiver and/or referral source to develop service plans.

  • Provides team with social work expertise.

  • Helps maintain continuity of care among team members by providing communication, documentation, and support.

  • Encourages enrollment by maintaining contact with caregivers, referral sources and potential clients as well as site liaisons and site managers from other sites.

  • Maintains client confidence and protects operations by keeping information confidential.

What you need to succeed!
