Summary This position is responsible for the delivery of social work services to identified home care clients. The Social Worker provides support and counseling to referred clients and facilitates financial and long-term care planning, as needed. POSITION SUMMARY: Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. The Master's Level Social Worker represents Holt in his/her community and ensures successful adjustment of child and family by conducting an assessment of the family, and providing appropriate services throughout the adoption process. The Master's Level Social Worker works cooperatively with families, the Branch Office and the Regional and Social Service Teams in Eugene, Oregon. Note: The Branch Social Worker is primarily a home based position. Collaborating and consulting with teachers, parents/guardians, district personnel, and service providers to find effective solutions to learning and behavior problems; always establishing and maintaining effective public relations and projecting a positive company image. Ensuring the use of positive social skill teaching methods by conducting monthly direct observation of classroom staff's implementation of IEP goals and/or Behavior Intervention Plans/ Treatment Plans. Providing guidance, consultation, and assistance to teachers in the development of classroom schedules, maintenance of student data systems and facilitation of student transition plans to assure appropriate schedules and services based on each student's Individual Education Plan (IEP) and/or other applicable goals. Assessing student progress through consistent review of classroom data collection and recording systems and providing applicable training and guidance to classroom staff accordingly in addressing student behavioral needs. Conducting crisis assessments and contacting the appropriate authorities if needed to ensure that emergency intervention is provided to students. Keeping up to date with research-based practices and developments in subject area. Responding constructively to formal and informal feedback. Performing other duties as assigned. ‖ Qualifications Required: Master's degree or higher in social work, educational counseling, therapy or a closely related field of study. Licensed currently or in the process of obtaining an IL professional educator license (PEL) school support personnel endorsement. Licensed currently or in the process of obtaining a licensed clinical social worker (LCSW) credential. Ability to obtain and maintain certification in company approved crisis management (PCM) training. Prior experience and/or knowledge in special education services and compliance, particularly for students with severe learning disabilities, emotional disturbance and/or behavioral disorders. Well-versed in applied behavior analysis (ABA) including positive behavior interventions and supports (PBIS), functional behavior analysis (FBA) and behavior intervention plans (BIP's). Proven success in developing and implementing effective intervention plans for students with diverse needs, preferably in an alternative, special education and/or behavioral health program setting. Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals. Ability to think and act quickly and calmly in an emergency and make independent decisions. Advanced oral and written communication, instruction, curriculum, conflict resolution, multitasking, problem solving, and decision making ability. Well-versed in the use of relevant technology including experience with computers, Microsoft Office Suite, database entry and basic office equipment. SummaryThis position is responsible for the delivery of social work services to identified home care clients. The Social Worker provides support and counseling to referred clients and facilitates financial and long-term care planning, as needed. The Social Worker is responsible for communicating with health care team and documenting all care activities.Essential Responsibilities/Accountabilities• Interprets to the home health team the social factors in the patient situation that may have bearing on his/her progress.• Helps patient and family understand and accept the illness and to anticipate its accompanying adjustments.• Coordinates resources to alleviate the financial impact of illness and assists patient and family in planning for payment of medical care.• Determines the need for continued social work service and draws upon other community agencies that can further assist the patient and family.• If necessary, assists with alternative planning for patients using appropriate community resources.• Assist with discharge planning from Lifetime Care program(s).• Serves as liaison with social work departments and other health and social agencies in the community.• Keeps social work staff informed of community and agency issues.• Consistently demonstrates high standards of integrity by supporting the Lifetime Healthcare Companies’ mission and values and adhering to the Corporate Code of Conduct.• Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures.• Regular and reliable attendance is expected and required.• Performs other functions as assigned by management.Minimum Qualifications• Bachelor’s Degree in health related field with a minimum two years related experience.• Home visiting experience strongly preferred.• Proven psycho social assessment skills.• Effective verbal and written communication skills essential.• Ability to stay organized and flexible important. Possession of a valid NYS driver’s license and own car required.• Requires pre-employment physical exam as required by NYS Dept. of Health.• Demonstrates strong communication (both written and verbal), interpersonal, organizational, and time management skills. • Basic computer skills are necessary. Experience with documentation in an electronic medical record • With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary.• Demonstrated ability to effectively manage time and caseload autonomously. • On Call may be required.Level II – require similar qualifications as level I, as well as:• Bachelor’s Degree in Social Work with a minimum of five years direct Social Work experience.
