This assessment is shared with the client and team members to assure that all relevant psychosocial issues are addressed in the care planning. Provide brief, culturally relevant, counseling to client/families which would include: crisis intervention; mental health concerns; pregnancy and birth options; grief; stress; parenting; coping skills; problem-solving; abuse/neglect; family violence.
