Role:

• Compensation: $300K+*

• 18 patients a day*
• Monday Friday work schedule*

• Small Patient Panel 450 patients max*
• Administrative time included in schedule*
• No nights or weekends*
• 100% outpatient*
• Dedicated Care and support team (have your own MA/Scheduler/Referral specialist)*
• Patient Population: Seniors with 3-5 chronic diseases*

Requirements:

• Residency Trained IM/FM*
• Board Certified/Board Eligible in: FM, IM or Geriatric Medicine
